Q: What is a lunch application?
A: A lunch application is a federal application to determine whether a student qualifies for either a free or reduced benefit when participating in the National School Lunch Program (NSLP).
Q: What happens when if there is incomplete information on the application?
A: Please be sure to complete all sections of the application. Incomplete applications will be returned and delay the process of a lunch determination. Unless you receive SNAP or TANF benefits then you must only complete the sections that indicate your child’s name, case number is required and sign the application.
Q: Who may fill out the application?
A: Only the parent, legal guardian, or the student if age 18 or older may complete the lunch application form. The application must be signed to ensure that the application will be processed.
Q: Can an application be completed online or do you prefer a paper-based application?
A: Lunch applications can be completed online through www.mymealtime.com or families can complete a paper-based application. We encourage you to complete it online as it will be processed faster.
Q: Do I still need to fill one out if I am not interested in the Lunch Program?
A: Yes, we request that a lunch application be submitted every year to your child’s school.
Q: When do I complete this form? When is the form due? What sections do I need to complete this form?
A: The application must be completed at the beginning of the school year and is due at that time as well. Once your application is received, the status is determined whether your child qualifies for FREE or REDUCED breakfast and/or lunch. All newly enrolled students will automatically have to pay the full price for breakfast and lunch until a status is determined.

A: For those who are interested in participating in the lunch program, you are required to fill out completely Sections 1-5. For those who are not interested in participating, please complete only Section 1 and 5. Please be sure in Section 4 to write NOT INTERESTED or DO NOT QUALIFY.
Q: If I have more than one child in the school are families required to complete multiple applications?
A: No, only one application per household is required. If you have children at different school campuses, please be sure to include them in the same application. Please make sure all members of your household are listed as well.
